Load Selection in Rehab: A Clinician's Framework
2d old
Most of what we know about load selection comes from strength and conditioning. Percentage of a max, reps in reserve, velocity. Every method was built and tested on healthy athletes trying to get stronger. Rehab borrows the whole playbook, and it doesn't transfer cleanly, because injured tissue is the one population whose baseline won't hold still. This is the framework I teach and use in clinic. It walks each method, where it breaks in a rehab setting, and what to trust instead when you can't test a max and the patient can't yet report effort. It covers where isometrics fit, why the next day tells you more than the session did, and how the margin for error narrows in injured tissue.
